4/14/26 just after 1 pm.: Patient is reaching out today. She received a Yellow Fever vaccine on 4/7 for travel consult. Today she is reporting the vaccination site is still red and sore and a little itchy. She denies any induration of the site or drainage. We discussed possibility of taking some OTC allergy medication to help with the itching and local site reaction. She also states that her arms and legs have been feeling heavy and they fall asleep very easily since receiving the vaccine. Patient then mentioned that when she was a child she had Mono and some markers for Guillan Barre. Patient says she might also be retaining more fluid than usual in her legs and has started to use her compression stockings to see if that might help.

Documentation 4/14/2026 Health Infectious Diseases MD Infectious Diseases Progress Notes MD (Physician) Infectious Diseases ID Brief Note: I called patient and she did mention no fevers or chills or cough or dyspnea. Some small amount of numbness and tingling in her extremities but overall minimal. She is able to walk around perform her regular activities. No weakness at all. I discussed with patient that sometimes there are side effects from yellow fever vaccine most are usually self-limited. My nurse will give a call around 4 PM. If symptoms of numbness and tingling persist I recommend going to the ER where I recommended lumbar puncture spinal fluid be sent for yellow fever PCR as well as IgM. I do not recommend staying at home overnight if she has persistent numbness and tingling of her hands and feet and prefer she will be in a monitored setting overnight although treatment is supportive care Additional Documentation Vitals: LMP 03/12/2026 Encounter Info: Billing Info, History, Allergies, Reviewed This Encounter, Eye Exams Encounter Status Electronically signed by MD on 4/14/26 at 14:05
