Symptom narrative

No adverse effects were reported; Nurse called to report that GARDASIL 9 was inadvertently administered to a patient after being improperly stored.; It was shipped on April 7, 2026 and did not arrive until April 13, 2026, and the distributor told the office it was not stable to use; This spontaneous report was received from a Nurse (registered nurse [R/N]) and refers to a 19-year-old female patient. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 13-Apr-2026, the patient was inadvertently vaccinated with an improperly stored Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) Suspension for injection (valid lot #A003663, expiration date: 11-Dec-2026) 0.5 mL administered for prophylaxis (Product storage error). Suspect vaccine was shipped on 07-APR-2026 and did not arrive until 13-APR-2026, and the distributor told the office it was not stable to use (Product distribution issue). It was however administered before they received the notification. No adverse effects were reported (no adverse event).
