Client here with mom and dad for scheduled immunization appointment. Client scheduled for Prevnar 20, Varicella, and MMR II. Client has no known drug allergies, no adverse reactions in the past to vaccinations, and parents report child not being sick at time of vaccination. Vaccine information sheets given to parents, side effects explained. RN, prepared immunizations. Client received Varicella and Prevnar 20 without issue. Client tolerated immunizations as expected. When this RN was taking the label off of the MMR vial for documentation, RN noticed that the sterile cap was still on the vial. It was at this point when this RN realized that the MMR vial was not reconstituted with the sterile diluent, and the sterile diluent was given alone without the MMR reconstituted in it. When RN realized the error, she spoke with the Public Health Manager. RN completed VAERS form. Health Officer and Medical Advisor made aware. Public Health Manager called client?s mom to explain the situation. Copy of VAERS will be given to client. Client to return to clinic later this week to receive MMR vaccination.
