Vaccine administration error: At 15-month well visit, child received ProQuad (measles, mumps, rubella, varicella vaccine) plus an additional MMR dose in error, instead of the intended separate MMR and varicella vaccines. No adverse effects were observed or reported yet to the primary care office. Parents have not been accessible via phone call at the time of this report, approximately 3 hours after administration. Medication administration error was detected at the time of reconciliation of vaccinations administered over the course of the day.
