Medication error/ adult dose of [Hepatitis B Vaccine (Recombinant)] was given to a patient instead of a pediatric dose; This non-serious case was reported by a consumer via call center representative and described the occurrence of wrong vaccine administered in a 13-year-old female patient who received HBV (Engerix B adult) (batch number LZ5X7) for prophylaxis. Co-suspect products included HBV (Engerix B pediatric) for prophylaxis. On 19-MAR-2026, the patient received Engerix B adult. The patient did not receive Engerix B pediatric. On 19-MAR-2026, an unknown time after receiving Engerix B adult the patient experienced wrong vaccine administered (Verbatim: Medication error/ adult dose of [Hepatitis B Vaccine (Recombinant)] was given to a patient instead of a pediatric dose). The outcome of the wrong vaccine administered was not applicable. Additional Information: GSK Receipt Date: 07-APR-2026 The reporter stated that an adult dose of Hepatitis B Vaccine (Recombinant) was given to a patient instead of a pediatric dose. The batch number was not provided and a request for this information has been made.
