A Regional Nurse Manager was seeking information regarding the validity of a PENTACEL dose. The caller mentioned that the DTaP-IPV component was not used/administered. Instead, the Hib component was reconstituted with sterile water and was administered on 11Mar2026. Call was disconnected prematurely while obtaining event information. Placed outbound call on 01Apr2026 at 03:10 PM. Reached voicemail, provided contact number and hours of operation. Additional Description of event Adverse events :Vaccines AE Reporting form not utilized, as the caller had limited information regarding the event. The call was also dropped prematurely. The only information available was that the vaccine was administered on 11Mar2026, on the left thigh intramuscularly. Lot number and expiration date were unknown to the caller.
