Here with mom for nurse only visit/ vaccine only for MCV and HPV, After vaccine administered, while sitting on table she passed out Mom and MA was present when she passed out. She was gently eased on exam table. A cool clothes was placed on her forehead . when the nurse came in room patient was laying on exam table and was conscious and talking. Was pale in color. While laying on table she had a lolly pop in her mouth. Bp was checked and noted to be 132/89, HR 62. No dizziness, headache, or nausea noted. At 1450 blood pressure was rechecked and noted to be: Bp 116/74, HR 69 . At 255 patient was sitting on table BP 109/68 HR 82. No dizziness, headache or nausea. Did check with Dr. Doctor reviewed BP and noted Bp to be fine. To have her stand to make sure still feels ok. Had patient stand by table no dizziness, nausea. BP 112/69 HR 88. Ok to go home. Escorted mom and pt to car. followed up with mom next day to check on patient. mom noted that she was fine.
