Patient called on 4/16/26 at 11am to report that she woke up the day after he second Shingrix vaccination with a reddened, inflamed injection site that was hot to the touch. She also reported swollen axillary lymph nodes on her left side (injection side). She said she later broke out in a rash on her right cheek that had the appearance of "broken capillaries". In addition, she reported having blisters on her tongue and possibly down her throat, that are red and painful. Patient said all seems to be starting to subside except for the lymph nodes, which continue to worsen. She spoke to her primary, who advised her to take Pepcid and an antihistamine to lessen the rash, then report her symptoms to me if she did not improve. She was not having any s/s of anaphylaxis. I suggested she try a cold compress, pain reliever, and possibly Benadryl for itching if her PA recommended it.
Pt reported that she was diagnosed with a heart condition (myocarditis or pericarditis)/ Multisystem Inflammatory Syndrome (MIS-A or MIS-C) after an infection with the virus that causes COVID-19.
Poss auto-immune condition.
