Symptom narrative

Shortly after receiving her first dose of the HPV vaccine, patient started to feel bad and laid down. Dad reports she then had several seconds of convulsions and looked very pale. He immediately got up and asked for help. LPN went in the room and reported that patient was supine, alert and oriented but very pale. She got a set of vitals and got her some crackers and juice. I went to go check in on her at this point and she was alert, oriented, sitting up and talking. She said she doesn't remember the episode but said she still felt a little shaky right now. I discussed that vasovagal reactions are a known reaction to the vaccine but seizure activity was not. Dad said it did look like a seizure to him but nobody else was there to witness the episode. I discussed that we will be reporting it to VAERS. Her vital signs were stable at this point but I did offer an evaluation at the emergency room if patient or dad desired a higher level of care as that would be a reasonable request. I told them I would be happy to facilitate a transfer if that is what they wanted. Patient asked for a cup of water and for some more time. She was given water and was allowed more time in the exam room. She then felt stable enough to leave on her own and declined an ER visit at this time. Her vitals were taken one more time and she left in stable condition. Dad felt comfortable with this plan.
