Symptom narrative

The patient started to feel her left arm itch at the Varicella vaccine administration site on 04/15/2026 around 6:00 PM. The patient noted redness around the vaccine administration site as well as raised skin. The patient contacted the clinic to report the adverse reaction on 04/16/2026 and was instructed to come into the clinic to be evaluated. The patient's arm was evaluated and the affected skin around the vaccine administration site was measured. The patient was instructed to contact her primary care provider to inform them of the vaccine adverse event.
