As of 4/15/2026, the patient was 27.4 weeks pregnant (EDD 7/11/2026). The provider signed an order for Tdap and Heplisav B. This MA did not verify if the Hep B needed was the Recombivax versus the Hepislav. Once the vaccines were administered, I noticed that the information was not cross matching and informed the provider. Once the provider attempted to place the order for Recombivax, it would not allow the provider to sign as the patient was days from her birthday. I contacted individual to discuss the event, and she assisted in contacting another source to make sure that all matters were addressed. 1) Dr. contacted the patient to inform her of the event. 2) Administration was documented under historical admin so that it would be documented as being given in the system. 3) Document and VAERS placed.
