Symptom narrative

Patient arrived at pharmacy for vaccine requesting vaccines. He stated he had not received an updated pneumonia vaccine. Patient immunization registry and immediate pharmacy file did not indicate a pneumonia vaccine in last two years. The following morning, clinical team reported patient had received the Prevnar20 vaccine 2 years and 2 months prior. Patient was consulted and has not experienced any side effects at this time. Reporting due to extra vaccine given outside CDC and ACIP vaccination guidelines.
