The patient's pertinent medical history, drug allergies or reactions, and concomitant medications were unknown by the reporter. On 15-Apr-2026, the patient was accidently vaccinated with a pediatric dose of Hepatitis A Vaccine, Inactivated (VAQTA) 0.5 ml (route of administration and anatomical location not reported, lot number Z010429, which was verified to be valid; expiration date established on 09-Oct-2026) for prophylaxis. (Product administered to patient of inappropriate age). There was no adverse event reported.
