the patient was inadvertently administered MMR II instead of PROQUAD.; No additional AE/ no PQC; he patient was inadvertently administered MMR II instead of PROQUAD; This Spontaneous report was received from Nurse and referred to a patient of unknown age and gender. The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 10-Apr-2026, the patient was vaccinated with the first dose of with measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live powder and solvent for solution for injection (M-M-R II) 0.5mL, lot Number: z005569, expiration date: 05-Mar-2027 (Strength, route, and anatomical location were not provided) for prophylaxis instead of MMR and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (Interchange of vaccine products and Wrong product administered). It was confirmed that there was no product confusion with labeling or appearance and that the nurse simply grabbed the wrong vaccine resulting in a medication error. No adverse events reported.
