Symptom narrative

gave only the liquid portion of the vaccine to a patient; gave only the liquid portion of the vaccine to a patient/ underdose; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old male patient who received Men ABCWY NVS (Penmenvy) (batch number 5S723) for prophylaxis. On 09-APR-2026, the patient received Penmenvy (right deltoid). On 09-APR-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: gave only the liquid portion of the vaccine to a patient) and inappropriate dose of vaccine administered (Verbatim: gave only the liquid portion of the vaccine to a patient/ underdose). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
