Administration after expiration date; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of expired vaccine used in a 6-month-old male patient who received DTPa-HBV-IPV (Pediarix) (batch number PE24L, expiry date 11-APR-2025) for prophylaxis. On 14-APR-2025, the patient received Pediarix. On 14-APR-2025, an unknown time after receiving Pediarix, the patient experienced expired vaccine used (Verbatim: Administration after expiration date). The outcome of the expired vaccine used was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 07-APR-2026 On 7 APR 2026, a pharmacy student called to verify the expiration date of Pediarix (lot number PE24L). The vaccine had been administered to a patient, and the pharmacy student believed that an incorrect expiration date had been documented. This was the patient's third dose of HepB, second dose of Tdap, and second dose of polio vaccine.
