not sure if she did or did not receive the second dose; This non-serious case was reported by a consumer via call center representative and described the occurrence of incomplete course of vaccination in a female patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Previously administered products included Shingrix (received first dose on 20-MAR-2019 with batch number 21546). The patient received did not receive the 2nd dose of Shingrix. The patient had incomplete course of vaccination (Verbatim: not sure if she did or did not receive the second dose). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 08-APR-2026 The patient was not sure whether she had received the second dose of Shingrix. Reporter stated that dose 2 may or may not have been given on 19-JUL-2019, there was no lot number on record for that dose, nor was there a record of which arm. The pharmacy where that dose was thought to have been administered had gone out of business, dose 2 was not documented in the state vaccination database. The patient was waiting for a call from her doctor about whether she would receive another Shingrix dose.
