Symptom narrative

patient has been in and out of hospitals for the past 2 weeks or so according to her. She received MMR vaccine at HD after feeling better from bronchitis on 4/6/26. However, she did end up in another local hospital on 4/12/26 with worsening pneumonia and acute toxic encephalopathy and sent to Hospital. At that time, HD was notified about patient as they were also ruling out TB disease. Patient does not have TB, and was discharged home to recover on 4/16/26 with antibiotics for pneumonia.
