Symptom narrative

HPV vaccine was given while the child was seated on the recliner chair. Child experienced brief unconsciousness about 2 minutes after the vaccine was given. She then regained consciousness after being patted on her hand. Ice pack was put on the back of her neck and was given to drink juice and some food which the child tolerated. Vitals were stable. No rash or itching was observed. Later went home but parents brought back the child to ER about an hour later thinking she had a rash on her cheek. She was evaluated at ER and sent home the same day and was advised to follow up with primary.
