Symptom narrative

Client reports a tingling sensation on tongue, throat tightness, shortness of breath, swelling of throat and tongue within 30 minutes of vaccine administration. Client was still breathing and speaking. Client was administered vaccine around 10:45 and was administered epinephrine and 50mg Benadryl by the pharmacist at 11:14. 911 was called and paramedics arrived around 11:20. Client was advised by paramedics to go to the ER, and client chose to have family drive her to the ER. Pharmacy recieved new prescriptions for prednisone and epinephrine that afternoon.

Allergies

None reported. Pharmacy will now be adding Shingrix to that list. Client does report history of allergies to snakebites.
