DTaP vaccine was administered to a 13-year-old patient; RMA called to report that a DTaP vaccine was administered to a 13-year-old patient instead of Tdap; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate age at vaccine administration in a 13-year-old male patient who received DTPa (Infanrix) (batch number Y97N7, expiry date 31-AUG-2026) for prophylaxis. Co-suspect products included DTPa (Reduced antigen) (Tdap Vaccine) for prophylaxis. Previously administered products included DTaP vaccine (received five prior doses of the DTaP vaccine, with the last dose administered at 4 years of age). On 09-APR-2026, the patient received Infanrix. The patient did not receive Tdap Vaccine. On 09-APR-2026, an unknown time after receiving Infanrix, the patient experienced inappropriate age at vaccine administration (Verbatim: DTaP vaccine was administered to a 13-year-old patient) and wrong vaccine administered (Verbatim: RMA called to report that a DTaP vaccine was administered to a 13-year-old patient instead of Tdap).
