Wrong diluent to reconstitute; Wrong diluent to reconstitute; This non-serious case was reported by a other health professional via call center representative and described the occurrence of wrong solution used in drug reconstitution in a 16-year-old female patient who received Men ACWY-CRM NVS (Menveo) (batch number AMVB079A, expiry date 31-MAY-2026) for prophylaxis. On 10-APR-2026, the patient received the 1st dose of Menveo. On 10-APR-2026, an unknown time after receiving Menveo, the patient experienced wrong solution used in drug reconstitution (Verbatim: Wrong diluent to reconstitute) and inappropriate dose of vaccine administered (Verbatim: Wrong diluent to reconstitute). The outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were not applicable.
