Symptom narrative

Spoke with mother of patient on the phone regarding reaction. Mother of patient wanted to know which vaccines were done in which leg. Informed mom that this leg rec'd the DTAP. Mother states the spot is lighter in color but still growing. Pt was taken to urgent care to be evaluated who states they were told it was an allergic reaction and was given Zyrtec. Mother states pt is acting normal. Consulted with Dr Pharm D and Dr attending . Description of spot: redness and swelling on vaccine site. Now the center of reaction is clearing and around is still red. Consulted provider who informed pt to take allergy med and continue to monitor.
