2/9 - pain at injection; drop of blood pressure 15 minutes after injection; 2/10-2/16: fever first day after injection, pain at injection; decreased range of motion; 2/16 - urgent care visit as pain was impacting sleep; prescribed 50mg methocarbamol (3x/day as needed), 375mg naproxen (2x/day), & patient was told they could supplement w/ acetaminophen for symptom relief; 2/17-3/2 - continued symptoms (pain in left shoulder / upper arm, reduced range of motion); 3/3 - orthopedic visit & x-ray, renewed prescription for napoxen, now at 500mg; 3/4-3/15 - continued symptoms (pain in left shoulder / upper arm + reduced range of motion) and side effects from naproxen: lightheadedness, dizziness; 3/16 - MRI; 3/17-3/31: continued symptoms; 4/1 - ultrasound-guided steroid injection and diagnosis of calcific tendonitis
