SIRVA - left shoulder. 04/16/2026: Pt presents re: "I had a routine physical exam back in March and received a tetanus shot during my visit. Strangely, the area of muscle that I got the shot in is still incredibly tense many weeks later. Not the tightest sensation that you would have in the first few days, but never a full recovery. I?m having difficulty raising the arm above my head without straining to get the muscle to work. Exercising a full range of motion still seems to leave my arm much more sore and more quickly sore than compared to my other arm. In my recovery, I did continue to use both arms as much as possible throughout tasks. From my experience, using the arm that received a shot and stretching usually aids in shortening the recovery period. I wanted to reach out to see if this was normal or if I should consult more advice on what is happening? I?ve had other shots in my life and have never experienced a recovery close to this time frame with basically no change from day 3 and on." The patient presents with persistent shoulder pain following a tetanus injection. Initial post-injection soreness improved over the first few days, but activity-related pain has continued. Reports pain with overhead lifting and reaching across the body; lifting bankers boxes at work recently increased soreness and fatigue, prompting the patient to stop after several boxes. Can raise the arm fully but pain increases exponentially with continued elevation and strain. Describes occasional weakness with use and soreness that develops quickly during activity; minimal to no baseline resting soreness. Notes a small tender area near the presumed injection site with deep pressure, but no palpable lump was ever appreciated by the patient or partner. He tried stretches and muscle flexing exercises without improvement. First night after the injection included difficulty sleeping; thereafter sleep improved. Location of pain includes the shoulder and extends halfway down the arm. DX: Shoulder injury related to vaccine administration (SIRVA) s/p TDAP administration 03/02/2026 resulting in tendonitis at the lateral margin of the acromion vs possible subdeltoid bursitis. - Start a course of oral anti-inflammatory medication for 2 weeks - Refer to physical therapy (Physical Therapy) - Advise optional icing for comfort if it feels helpful; no requirement given time from initial injury. Outcome pending.
