he is not sure if he did or did not receive the second dose; This non-serious case was reported by a consumer via call center representative and described the occurrence of incomplete course of vaccination in a male patient who did not receive Herpes zoster (Shingrix) for prophylaxis. Previously administered products included Zostavax (received Zostavax on an unknown date) and Shingrix (received first dose on 20-MAR-2019 with batch number 21546 in an unspecified arm). The patient did not receive the 2nd dose of Shingrix. The patient had incomplete course of vaccination (Verbatim: he is not sure if he did or did not receive the second dose). The outcome of the incomplete course of vaccination was not applicable. Additional Information: GSK Receipt Date: 07-APR-2026, 08-APR-2026 Patient got his COVID shot the previous week of reporting, the pharmacist asked if he wanted to get the second Shingrix vaccination. The pharmacist brought it up because the state vaccination record showed only the first vaccination, which he had received in March 2019. Patient vaguely remembered returning in July 2019 to get the second Shingrix, but he might have been mistaken. Patient got all the relevant information for the first vaccination but had not sent information on the second one, so it was possible he had never received it, his insurance for 2019 had also told him it had no record of either vaccination. To be safe, he wondered whether he should get another Shingrix vaccination then, or whether there would be a safety problem if it turned out to be a third dose. The batch number was not provided upon follow- up with the reporter.
