No side effects or symptoms; was supposed to give VARIVAX but accidentally gave VAXELIS; This spontaneous report was received from a Other health professional (Medical Assistant) and refers to a 23-year-old male patient. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 17-Apr-2026, the reporter was supposed to give Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) but accidentally was gave Diphtheria and Tetanus Toxoids and Acellular Pertussis Vaccine Adsorbed, Inactivated Poliovirus, Haemophilus b Conjugate [Meningococcal Protein Conjugate] and Hepatitis B [Recombinant] Vaccine (VAXELIS) prefilled syringe (lot #U8512AA, expiration date: 30-Apr-2028) 0.5 mL to the patient (Wrong product administered) for Prophylaxis. No side effects or symptoms. Customer did not have any medical questions just wanted to report it. No AE/PQC.
