Symptom narrative

Patient fainted in waiting room after receiving vaccine, slumped to floor with brief LOC, no head injury, pt was laid flat with knees up then moved to seated position and then to chair, bp was rechecked and measured 99/62, given a lollipop, water and juice and sat for about 5-10 minutes until he was able to stand and walked to car with dad.
