Symptom narrative

Patient received vaccine that was later identified to have been stored outside of recommended temperature range due to refrigerator monitoring configuration failure. Vaccine storage unit was found to be out of range (~13?15°C) from 4/8/26 through 4/21/26. Alert system was not active due to incomplete configuration at time of installation, resulting in delayed identification. At time of administration on 4/13 @ 1430, recorded temperature was approximately 14 degrees C. No adverse reaction reported at time of filing. Report submitted due to potential vaccine storage excursion and per safety reporting guidelines.
