No symptoms were reported. No additional AE.; Medical assistant report administration of improperly stored vaccines to multiple patients. No symptoms were reported. Original AE was filed in case # 02985207 without any AE details. See additional AE filed in case numbers 02985477; 02985543; 02985; This spontaneous report was received from a medical assistant and refers to a 1-year-old patient of unknown gender. The patient's pertinent medical history, concurrent conditions, previous drug reactions or allergies and concomitant medications were not reported. On 15-Apr-2026, the patient was vaccinated with Hepatitis A Vaccine, Inactivated (VAQTA), lot #Z005021 has been verified to be a valid lot number, expiration date: 22-Oct-2026, administered as prophylaxis (dose, route of administration and anatomical location were not reported). The vaccine administered was improperly stored, undergoing a temperature excursion above 9°C (14.9°C) for 298 hours and 0 minutes (Product storage issue). No symptoms and no previous temperature excursions were reported (No adverse event).
