Symptom narrative

Fussiness within 5 hours after vaccination. Was co administered with Pentacel, Prevnar, and Hep B. 1 day after in the AM, fussiness was resolved but patient has scant bright red blood in stool.

Reflux. Suspect Milk protein allergy. On Nutramigen exclusively for > 1 month.
