Pt noted onset day of vaccination (04/16/26) to the right arm of recurrent T103F, chills, sweats, migraines, aches, fatigue and sharp electric pain in the upper left side of my chest. She defervesced on day + 3, however her left left arm was getting heavy and hurt (4/18/26) so patient visited ED. She denies SOB, nausea.At ED, Vitals WNL. Exam WNL. CXR WNL. ECG without features of pericardial effusion. Labs only remarkable for mildly reduced platelet count 134, coags for INR 1.2, PTT 13.5; otherwise troponin, WBC count, CMP, troponin I WNL, ESR WNL, HCG negative. DDx pericarditis, costochondritis, pleurisy, pneumothorax, acute coronary syndrome. Discharge diagnosis was mild pericarditis. Pt discharged on medrol dosepak and ibuprofen 600mg Q6hrs. PCP referred to allergy consultant for question of risk of recurrence upon future boosters vs contraindication.
