While checking on-hand inventory for Priorix on [date discovered] due to a scheduled administration, an additional vial of Priorix powder was identified. This prompted a review of recent administration records and available camera footage. Upon review, it was observed that on 09/19/2025, a patient presented for three vaccinations, one of which included Priorix. Camera footage shows the pharmacist retrieving three vaccines; however, no separate vial required to reconstitute the Priorix with diluent was retrieved. The pharmacist then attached three needles to the vaccine syringe barrels and entered the immunization room. This finding suggests the possibility that Priorix may not have been properly reconstituted prior to administration.
