Symptom narrative

Patient received HPV vaccine on 4/16/2026 at 1:45 pm and shortly after vaccine was given in right deltoid muscle via IM, patient proceeded to check out and started experiencing a change in color ( pale around the lips and face). Patient also expressed decreased hearing in both ears. Patient was brought back to room. Blood pressure was obtained and was 92/41 via right arm. Patient assessed and no fever or loss of consciousness noted. Patient placed in supine position. Pulse oximeter read 98% on room air, no respiratory distress was noted respirations remained consistently between 22 and 24 bpm. No drooling or increased secretions or signs of anaphylaxis noted. After 15 minutes of monitoring patient closely his facial color returned to normal appearance. Blood pressure obtained was 94/63. Patient verbalized feeling better and could walk without dizziness. No ER or hospitalization required. Vaers report was completed and filed to cdc. Follow up call to parent 4/21/2026. Mother stated that patient was doing ok and no recurrence of adverse symptoms. Mother stated no further assistance needed. Parents declined future HPV vaccine administration. LAST VISIT IN THIS CLINIC: 10/27/2025 NEXT APPOINTMENT SCHEDULED IN THIS CLINIC: No upcoming appointments scheduled.
