Co-suspect products included Herpes zoster (Shingrix) for prophylaxis. The patient's past medical history included chickenpox (had in past). Concurrent medical conditions included hashimoto's disease and allergy. Concomitant products included cetirizine hydrochloride (Zyrtec) and levothyroxine sodium (Tirosint). On 10-APR-2026, the patient received the 2nd dose of Shingrix (left gluteal) and the 1st dose of Shingrix (left arm). On 10-APR-2026, an unknown time after receiving Shingrix and not applicable after receiving Shingrix, the patient experienced overdose (Verbatim: administrator at the time as to if it was correct to receive 2 doses at one visit.) and vaccine administered at inappropriate site (Verbatim: second dose in her left gluteal). On 14-APR-2026, the patient experienced felt faint (Verbatim: The patient reports that she felt like she would faint this morning). The patient reports that she felt like a weakling. The patient states she had been sleeping 12 to 14 hours per day. The patient states that she feels spacey like a space cadaver at the time of reporting. The patient reports that her brain feels slow. The patient reports that she felt like she would faint on the day of reporting.
