A 72-year-old male patient received BNT162b2 (BNT162B2 NOS), in 2022 as dose 1, single (Batch/Lot number: unknown), in left arm, as dose number unknown, single (Batch/Lot number: unknown) and as dose number unknown (booster), single (Batch/Lot number: unknown) for covid-19 immunisation. The patient's relevant medical history included: "Heart disorder" (unspecified if ongoing), notes: he got turned down to go to Vietnam because of his heart.; "Constant Afib" (ongoing), notes: Constant Afib; "he can not remember anything" (unspecified if ongoing). The patient took concomitant medications. The following information was reported: LOSS OF CONSCIOUSNESS (hospitalization, medically significant) with onset 2022, outcome "not recovered", described as "passing out"; ROAD TRAFFIC ACCIDENT (medically significant) with onset 2026, outcome "unknown", described as "got into a accident he doesn't drive anymore"; VERTIGO (non-serious), outcome "unknown"; HEAD INJURY (non-serious), outcome "unknown", described as "when he hit his head". The event "when he hit his head" required emergency room visit. The event "passing out" required physician office visit and emergency room visit. The patient underwent the following laboratory tests and procedures: Electrocardiogram: Unknown result; Electroencephalogram: Unknown result; Lab test: Unknown result, notes: has had more tests. Therapeutic measures were taken as a result of loss of consciousness. Clinical course: The patient reported receiving Comirnaty (Pfizer COVID 19 vaccine) and stated that the onset of his symptoms began after he started receiving the vaccinations during the COVID 19 pandemic. He reported that he is older and has difficulty recalling specific dates or details. Over the past four years, he has experienced recurrent episodes of syncope, for which he sought medical evaluation; however, no definitive cause has been identified. He reported discussing his symptoms with a friend who is a retired firefighter, who indicated that they had observed similar episodes in individuals who received the Pfizer COVID 19 vaccine, leading the patient to suspect a possible vaccine-related side effect. The patient stated that he has experienced approximately 16 episodes of syncope over the past four years, occurring suddenly and without warning, including while walking outdoors. These episodes have been witnessed and can be verified by his wife and friends. He reported undergoing extensive medical evaluations, including EEG, EKG, and additional diagnostic testing, with assessments of both the brain and heart, none of which identified a cardiac or neurological cause. Approximately 3 to 4 months ago, the patient experienced a syncopal episode while driving home from the post office, resulting in a motor vehicle accident; he has since stopped driving. He also reported being afraid to walk due to the unpredictability of these events. The patient stated that his syncopal episodes began after initial COVID 19 vaccination, although he does not recall the exact date. He reported receiving 5 to 6 to 7 or more doses, including yearly booster doses, and stated that recently he has been receiving double doses due to his age. He noted that his medications have changed over the past few years but did not believe these changes contributed to his symptoms. During some episodes, the patient reported hitting his head; in those instances, he was evaluated in the emergency department and underwent imaging to rule out intracranial bleeding, with no abnormalities reported. He reported occasional overnight hospital observation. Vertigo was considered as a potential diagnosis; although the patient has a history of vertigo, it was not determined to be the cause of his syncopal episodes. The patient stated that he documents each episode on his calendar. The caller reiterated that the patient had never experienced syncope prior to receiving the Pfizer COVID 19 vaccine and stated that the episodes began approximately six months after the first vaccination. The patient's treating healthcare provider is aware of his condition but has been unable to identify an underlying cause. The patient did not receive any other vaccines on the same date as the suspect vaccine or within four weeks prior to the first vaccination. He reported no known allergies to vaccines, medications, food, or other products. The patient was referred to his treating healthcare provider for further evaluation.
