Our hospital (XXX Hospital in XXX) purchases vaccine both for the hospital and for associated clinics. We just transitioned to purchasing the prefilled syringe for Shingrix to help prevent medication errors where only the vaccine diluent is administered instead of including the vaccine powder. Upon receiving the new product, our pharmacy buyer noticed that the packaging is nearly identical to the prefilled syringes of Havrix. Both vaccines are from Glaxo Smith Kline. This is concerning for risk of error in clinic settings were barcode scanning is not utilized. Look-alike sound-alike packaging. Our clinics will be notified of the LASA risk and advised to store these vaccines in separate areas within their refrigerators. Supply within the central pharmacy will also be separated to prevent mixup of vaccine products.
