Symptom narrative

The patient received two vaccines, Hepatitis A and Typhoid fever, on 04/16/2026 at around 4:30pm for his upcoming trip. That same day, patient had taken one dose of ondansetron 4 mg and one dose of meclizine 25 mg due to benign paroxysmal positional vertigo. Later that evening, around 9pm, the patient experienced a rash on his trunk (from nipple line down to groin on both front and back of body). Patient described the rash as small, itchy, red bumps. He did not take any medications to help clear the rash, and it resolved on its own in a few days. He did not take any more doses of ondansetron or meclizine. Patient did not tell any medical providers about the rash until coming back to the pharmacy on 04/22/2026, where he told the pharmacist who vaccinated him.

Medical history

Gout, dyslipidemia, hypertension, chronic kidney disease, osteoarthritis, hearing loss/tinnitus

Tamsulosin 0.4 mg daily, omeprazole 40 mg daily, losartan 100 mg daily, atorvastatin 10 mg daily, celecoxib 200 mg daily, Centrum Silver multivitamin. Patient also reported taking one dose of ondansetron 4 mg and meclizine 25 mg for the fir
