Symptom narrative

Pt reported over the phone that he believed he was having an adverse reaction to the Tdap he had received. He reported having had Tdap vaccinations multiple times in the past without issue. He was given his vaccination to the left deltoid. He reports that for 2 days after the vaccine he had some redness and swelling in the area. This resolved but he had pain that remained at the vaccine site until about 4/13/26. He also noted that he had had intermittent tingling throughout the entire arm since receiving the vaccine. This progressed to intermittent numbness and tingling specifically to digits 4 and 5 of the left arm from 4/13/26-4/15/26. He denied any weakness to his grip. However he works out regularly and states that on 4/14/26 in the gym when doing a tricep extension although he was able to use the same amount of weight he was able to do less reps than usual. He has never had anything like this before, he denies any trauma to his neck, arm, elbow or wrist. He denies any pain to his neck, arm, elbow or wrist. He denies any new activities or hobbies that may be contributing to his symptoms. He had tried ice and ibuprofen which has helped somewhat. Pt advised to see a Designated Medical Provider as per our Occupational Health protocol. A follow phone call was placed on 4/21/26 and patient reported feeling 65% improved. He had not yet established care with Designated Medical Provider for this incident. He was again encouraged to do so. We have had no further follow up with this patient.
