Symptom narrative

Patient was in clinic today for 2 yr Well Child Check. At time of visit, patient is due for 1st dose Hep A. MA collected the vaccine from vaccine fridge, verified patients name/dob. Proceeded to exam room. MA advised would be best to have daughter sit on her lap to give vaccine. MA left the room, injection given, patient tolerated well. MA pulled up immunization in patient's chart, proceeded to scan the bar code and received an error stating no vaccine on file. In looking at the vaccine were located in the vaccine fridge noticed that the Hep A pediatric was in the wrong spot, was actually where the Hep B pediatrics belongs. Changed the vaccines back to correct location. Advised MA Supervisor of error right away. Also notified the Provider of error
