Symptom narrative

Patient waited in room for 15 minutes after receiving vaccines. She got up to leave the clinic and after getting to the hallway, began to feel faint. She sat down and we monitored her vitals. Her blood pressure was initially at 98/62 and after waiting for 10 minutes rose to 108/70. She stated that she felt fine and wanted to go home. She was able to stand up and get into a wheelchair. Patient did not lose consciousness and was transported by wheelchair to her car. Mother had not decided whether or not to take patient to the emergency room.
