This writer went in to administer the 4 month WCC vaccines to patient, which should have been Rotarix, Vaxelis, and Prevnar. Writer first administered the Rotarix by oral route while patient was in dads arms. After Rotarix was administered we moved patient to the exam table to administer the other 2 vaccines. Writer then administered the Gardasil to the right thigh, as soon as this writer injected the Gardasil, writer realized it was not the Prevnar that should have been administered. Writer then exited exam room and went right to the provider to let her know what had transpired. The provider spoke with Poison Control for reactions. Provider then went in to let the family know that the wrong vaccine was administered. In this case the patient should have been given Prevnar, but was given Gardasil by mistake. Provider discussed monitoring parameters with parents and plan for follow up in the office 2 days from the incident. Internal incident report and investigation ongoing.
