Symptom narrative

On 4/20/26 clinic nurse administered Fluzone "high dose" vaccine to 28-year-old patient. High dose recommended for 65-year-old and older. Identified only when entering vaccine bar code into EHR Patient notified; Health Officer notified = no further actions indicated. Extra precaution must be exercised due to significant similarity of vaccine boxes. Locate separately with clear identifications.
