Spoke with client via phone on Thursday, 4/23/26. Client reports experiencing possible neurological side effects following recent vaccinations received on Monday, 4/20/26. She describes symptoms of confusion and difficulty sleeping since vaccination. Client stated: ?I have not gotten much sleep since I received the vaccines on Monday. Monday night I slept for maybe two hours and Tuesday and Wednesday night for maybe three hours each. I have also been forgetful for the past two days. This morning I was waiting for a train and noticed the one coming was not my train. I then looked down, and when I looked back up, it was like I had never seen the train before, and I got on it.? Client denies additional symptoms including headache, neck pain, or other concerns at this time. Client reported she was planning to seek evaluation at urgent care but wanted to consult with our office first. Strongly encouraged client to seek immediate medical evaluation at urgent care/emergency department due to neurological symptoms. Client verbalized understanding and agreed to go for evaluation and provide an update.
