Reported Symptoms: 10022086:INJECTION SITE PAIN; 10042680:SWELLING ARM; Narrative: per 5/20 nursing note "Patient into clinic with concern of possible reaction to the Meningococcal B vaccine; has swelling to left arm at injection site no redness to area, has used Tylenol & iced the area with no relief. PCP recommends to continue to ice the area to decrease the swelling" Per 5/27 nursing note "pt states can lift his arm not but still has pain is resting on it & cannot sleep to that side, denies redness to area or swelling, or fever." Per 6/5 nursing note "called to inquire how the pain is doing from vaccine injection, states it seems to be improving.
