Symptom narrative

Patient received vaccine on 4/3/26. Patient verbalized some discomfort during the injection but did not verbalize further pain after administration. Patient was seen 4/13/26 with complaints of shoulder pain described as "muscular but also around the shoulder joint". From the providers office note from 4/13/26, the patient states "that he has been bench pressing and playing basketball and shoots with his right arm. No known trauma. He states that the pain has been worsening to the point that he has difficulty raising his arm to wear his jacket".
