Symptom narrative

Patient is a 1 year 8 month old presenting for a wellness visit. Patient is due for Hep A and Dtap at this age. I took vaccines out of the refrigerator after getting immunization consent form signed by parent. I had a fellow MA verify the vaccines and at the time did not scan prior to giving them as there was someone standing at the scanner I was going to use, I generally scan prior to giving the vaccines. I did not, lesson learned. After giving the patient Hep B instead of hep A i noticed right away told my supervisor and told the parents of the event. They will return at a later date for the recommended hep A and watch for any adverse reactions.
