Symptom narrative

After vaccination at 04:00pm, pt started feeling very dizzy. He started sweating, was very pale, had blurry vision, difficulty hearing. Blood pressure was measured to be very low, approximately 70/40. I administered 1 dose of Epi-pen approximately 2 minutes after vaccine was administered, and continued to monitor the patient. His vitals improved and were stable until EMT arrived at 04:30p. They examined the patient and took him to the hospital for follow-up.
