administered an expired dose of Pediarix to a patient; This non-serious case was reported by a nurse via call center representative and described the occurrence of expired vaccine used in a 6-year-old male patient who received DTPa-HBV-IPV (Pediarix) (batch number X9EP5, expiry date 09-MAR-2026) for prophylaxis. On 15-APR-2026, the patient received Pediarix. On 15-APR-2026, an unknown time after receiving Pediarix, the patient experienced expired vaccine used (Verbatim: administered an expired dose of Pediarix to a patient). The outcome of the expired vaccine used was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences.
