Symptom narrative

Patient is a 13 y.o. female presents to the ED with witnessed syncopal episode. Patient has a history of ADHD oppositional defiant disorder resides at group home does have a history of head banging and frustrated and did do this yesterday. Today by EMS report she walked in from outside her witnessed syncopal event twitching but noted seizure activity and she has no history of this type of reaction either seizures or syncope in the past. On EMS arrival she was staring into space removing only the left upper extremity however after a short time became alert and oriented x4. My evaluation does remember hitting her head yesterday and has had a headache since this time which is not changed after this event today. It is described as posterior. She remembers playing outside and feeling hot in walking and class standing up and feeling dizzy before passing out. She does not remember being on the floor but remembers waking up in the ambulance. She now has the continued headache as well as right upper quadrant abdominal pain no nausea vomiting or diarrhea fevers chest pain shortness of breath. She does feel like her arms and legs are tingling and that her vision is blurry no difficulty speaking or swallowing or weakness or unilateral numbness or weakness. LMP 1 week ago denies chance of pregnancy in no dysuria frequency or urgency. No other complaints at this time.
